Manager of Care Management

The Manager of Care Management (MCM) is responsible for overseeing, developing, operationalizing, and managing all care management systems and processes. The MCM provides the leadership to ensure care management performance objectives are met, programs and resources are used effectively and execution of both short term and long term care management goals.

Responsibilities
Responsibilities include all aspects of promoting cost effective quality outcomes that are monitored across the care continuum and making decisions related to the provision of care. The MCM works collaboratively with physicians, health plans, hospitals, ancillary providers, and other partners and stakeholders to promote interdisciplinary teamwork that fosters goal achievement. The MCM is instrumental in the organization’s growth and development, and continuously works to evaluate program effectiveness and develop opportunities for enhancement. The MCM oversees, educates and trains the care management staff that deliver care management services in the acute and ambulatory settings, and differentiated services in various health plan programs.

Area of Responsibilities

  • Establish a culture of excellence in performance and innovation that seizes challenges and makes them opportunities to create new models and processes that better serve patients and their families.
  • Leads and manages Clinical Care Coordinators (CCC) by example demonstrating the capacity and expertise to meet ambitious but realistic goals.
  • Establish and monitor metrics and benchmarks to drive improvement and raise awareness that promotes individual growth and collective improvements in care.
  • Facilitates engagement and collaboration with community members to optimize care coordination across continuum of healthcare providers and service agencies creating an integrated and collaborative Medical Neighborhood.
  • Assess and assist patient centered medical home practices in incorporating the embedded care manager role as part of the care team.
  • Develops and evolves care management processes to include methods for: Determining program eligibility. Assesses patients encompassing social, physical, behavioral, and economic barriers to optimal care. Implementing evidence-based care processes to include disease-specific patient self-management care plans, medication reconciliation and adherence plans, and transition of care management. Monitoring, triaging, and discharging patients managed by programs. Measuring the performance of the program incorporating clinical, financial and patient experience indicators. Engaging patients in the management of their health and fostering adherence to care plans assuring adequate and effective program staffing.

 

Required Knowledge & Skills

Education: Bachelors of Science degree in Nursing, Public Health, or related field or equivalent combination of experience and education

Experience: Five years of related experience. Requires broad knowledge of complex systems and procedures

Licenses and Certifications

  • Georgia Registered Nurse License
  • Supervisory experience preferred. Proven success in teambuilding and training coaching staff to meet operational requirements and goal

 

Qualifications

  • A minimum of five years’ experience as a registered nurse and two years in care case management leadership role.
  • Ability to analyze and integrate information and make sound decisions based upon established guidelines.
  • Thorough understanding of health plan and regulatory rules, managed care, care management, disease management and referral processes.
  • Highly self-motivated, organized and detailed oriented with an attitude for continuous improvement.
  • Proven success in teambuilding, training and coaching staff to meet operational requirements and goals.
  • Must be able to prioritize, plan, and handle multiple tasks and demands simultaneously.
  • Highly professional with excellent written and verbal communication skills.
  • Proficient with computer software programs, including Microsoft Word, Excel, Visio, and Electronic Health Records.
